PROJECT 02 · PREPARED FOR LEADERSHIP REVIEW

Intake Excellence Initiative

A proposed practice-improvement initiative to strengthen intake communication, clinical review, documentation, handoff, and admission preparation.

Complex intake work depends on reliable handoffs.

Behavioral health referrals may originate from multiple sources and require coordinated clinical review, communication, documentation, and admission preparation. When information is not organized through one shared pathway, important details may be harder to locate or communicate consistently.

Why it matters

A clear, shared intake pathway may support patient safety, communication continuity, admission readiness, and more consistent clinical review.

One visible, standardized intake pathway

Receive referral→Clinical review→Acceptance communication→Nurse handoff→Prepare admission

My contribution / nursing leadership

Developing and organizing proposed workflow, communication, documentation, and decision-support resources for leadership consideration.

Preparation stage — implementation has not begun and organizational approval is not implied.

Standardize

Review one shared referral-to-admission pathway.

Digitize

Evaluate proposed APeX and information-access concepts.

Evaluate

Establish baseline measures before any pilot.

Current status

Materials prepared for leadership review. Clinical validation, APeX/Informatics review, implementation planning, and outcome measurement remain pending.

Reviewer note

These portfolio materials are presented for leadership consideration. Organizational branding within a source artifact does not indicate final policy approval, adoption, or implementation.

ADD VERIFIED TOOL
Escalation terminology to review

Some draft source artifacts reference “Nursing Supervisor.” Proposed Unit A escalation pathway for leadership confirmation: Manager → Director → Administrative On-Call, when indicated.

Potential value includes greater clarity, more consistent communication, better-organized intake information, and improved admission preparation.

OUTCOME MEASUREMENT PENDING

Potential benefits — evaluation pending. No time savings, error reduction, adoption, or implementation result is claimed.

  • Obtain leadership review of the proposed Intake Excellence Model.
  • Validate admission decision-support content and escalation language.
  • Request APeX/Informatics review of the proposed SmartPhrase and digital workflow.
  • Finalize the Nurse-to-Nurse Report tool.
  • Select a limited pilot scope and establish baseline measures before implementation.
